The Bounce Park Waiver

All persons entering and using The Bounce Park do so at their own risk. The promoter (Elite Bounce Ltd), the venue and any associated staff members take no responsibility for injuries or damages caused at The Bounce Park.

This document must be read, understood and agreed to before use of The Bounce Park. Any participants that have not agreed to the document, will not be allowed access to The Bounce Park. Participants under the age of 18, must have the document completed by a parent/guardian over the age of 18.

All children under the age of 5 must have a full paying adult bouncer on The Bounce Park with them at all times.

 

For participants 18 years and over

• I agree that upon entering and using The Bounce Park, I do so at my own risk.

• I agree that whilst undertaking the activity, I will do so in the accordance of the safety rules and advice provided. I will also follow any instructions or advice provided throughout the activity.

• I certify that I have read the safety rules provided before using the equipment.

• I acknowledge that I am responsible for my own safety whilst undertaking the activity.

• I understand that I am responsible for my own belongings whilst at The Bounce Park, including removing any phones, sharp objects and jewellery etc whilst using the equipment.

• I certify, to the best of my knowledge that I am good health and have no medical conditions which may have the effect of making it more likely that I will be involved in an incident which may involve injury to myself or others at The Bounce Park.

• I certify that I have no pre-existing medical conditions which, could through participation in the Activity, result in injury to myself or damage to my health.

• I certify that to the best of my knowledge I am not pregnant.

• I understand that I need to inform a member of staff of any accident/incident/illness at The Bounce Park, and give my consent for staff to provide medical attention if needed.

• I acknowledge that the maximum permitted weight for participation in the Activities is 20 stone and confirm that I do not exceed this weight.

• I give my consent for photos/videos to be taken on the day for promotional purposes.

• I understand that The Bounce Park Antibacterial socks must be worn at all times whilst using The Bounce Park these can be purchased on the Bounce Park tickets page.

On behalf of participants under the age of 18

I am the parent/guardian of the child/ren listed who is/are under eighteen (18) years of age. I consent to allow that/those child/ren to participate in in the use of the equipment at The Bounce Park. I declare that if I am not the parent or guardian of the child/ren I have authority from the child/ren’s parent or guardian to sign this risk acknowledgement form.

• I acknowledge that I am responsible for the child/ren safety whilst undertaking the activity.

• I understand that I am responsible for my child/ren belongings whilst at The Bounce Park, including removing any phones, sharp objects and jewellery etc whilst using the equipment.

• I certify, to the best of my knowledge that the child/ren are in good health and have no medical conditions which may have the effect of making it more likely that they will be involved in an incident which may involve injury to themselves or others at The Bounce Park.

• I certify that the child/ren have no pre-existing medical conditions which, could through participation in the Activity, result in injury to themselves or damage to their health.

• I certify that to the best of my knowledge that the child/ren is/are not pregnant.

• I understand that the child/ren need to inform a member of staff of any accident/incident/illness at The Bounce Park, and give my consent for staff to provide medical attention to the child/ren if needed.
